1. Define health disparity and identify a specific health disparity you identify in your area of practice: CARDIOVASCULAR DISEASE 2. Discuss the nine determinants of health – factors that influence health (WHO, 2010) and describe how they could influence the identified health disparity or problem. 3. Identify the epidemiology associated with the problem and specify the incidence at both the state and national levels. 4. Describe the cultural considerations for your identified problem. 5. Describe the health care literacy challenges with the population impacted. 6. Apply a health promotion theory to the development of a treatment plan for the condition. 7. The scholarly paper should be in narrative format, 6 to 8 pages excluding the title and reference page. 8. Include an introductory paragraph, purpose statement, and a conclusion. 9. Include level 1 and 2 headings to organize the paper. 11. Include a minimum of 4 professional peer-reviewed scholarly journal references to support the paper (review in Ulrich Periodical Directory) and be less than five (5) years old. 12. APA format is required (attention to spelling/grammar, a title page, a reference page, and in-text citations).
