YOU ARE TO DO THE TASK MARKED TASK 2 ONLY.
GUIDELINES FOR TASK 2
Task 2:
-Identify a community for which there is data available that will support the diagnosis.
-Provide an assessment of how each of the tools listed was used in the assessment of the identified community.
-Provide a logical interpretation of the collected data using concepts of epidemiology.
-Identify any problems based on the Healthy People 2020 goals for the selected community.
-Provide a logical discussion of the problems in relation to the Healthy People 2020 goals.
-Provide a logical discussion of community resources that are available to address the selected problem.
-Provide a logical primary prevention topic based on the problem identified in part C2.
Part A is a description of your community county (PRINCE GEORGE’S COUNTY).
For example, “The community assessed was my county in the state of MARYLAND. The region is ethnically diverse owing to a large migrant population. ” J
Part B. As for the assessment tools, you will want to write 2-3 paragraphs for each tool where you first state how you used the tool (i.e. examine physical and environmental features of the community that contribute to the health of community members). You will then list the data you collected. No need to include everything. For example, in the disaster assessment, you can describe the risks faced by your community, the disaster plan in place, and then discuss and issues you discovered (if you did). Be sure to use APA style in-text citations for all data and other unique information provided!
For part B1, you need to make a separate subheading “Interpretation of Collected Data/Other Epidemiological Data” Under this subheading, you will want to describe what you learned from the 6 tools and put the data in context. You’ll state epidemiological data for your community. Some of this is not asked for on the assessment sheets so this is your opportunity to discuss the major causes of morbidity and mortality, birth rates, death rates, etc. For this data, how does your community stand relative to the state National data Put your data in context!! Add meaning to the epidemiological data for your community using what you learned from the assessment tools!
Below is a brief discussion of epidemiology data. Your “Interpretation of Collected Data” section will be longer:
“My collected epidemiological data during the years …………… revealed that for PRINCE GEORGE County, including the town of RIVERDALE …………, morbidity was primarily due to high rates of cancer and heart disease with……….. .”
Part C1. Now, you should have a pretty good idea of the top 3 health concerns. These health concerns should be apparent following your assessment. Present them in this section. If you are having any trouble, you may want to use the approved topics list (www.countyhealthrankings.org can be helpful too) to help you determine which issues are significant in your community. As long as the health concerns you identify can be located within Healthy People 2020, you can use them for this section.
Part C1a. You will characterize each of these health issues in your community and then explain what Healthy People 2020 aims to achieve in relation to these three issues. Go to http://www.healthypeople.gov/2020/topicsobjectives2020/default and find each one in this glossary. Click on the topic (i.e. healthy nutrition and weight status for obesity). Check out the “Overview” and “Objectives” tabs once you open the topic. This is what you want to discuss. You should relate the status of this health concern in your community to the status and goals at the national level described by HP2020. Be sure to incorporate some specific objectives of HP2020 into your discussion.
Parts C2/2a/3. Finally, you’ll want to select one of these health concerns and expand upon it some more in terms of primary prevention. Discuss how agencies in your county are trying to improve this health indicator. Discuss how primary prevention efforts could reduce morbidity and mortality. For example, promotion of healthy nutrition and exercise would reduce obesity that is a known risk factor for heart disease and diabetes therefore reducing cardiac related deaths and associated health risks with diabetes. This section is like forecasting the future for C229, the field experience where you need to really dig in and learn about the services that help community members connect with health care services in the community!!
Part D. Provide sources in APA format.
