Behavior change paper

The paper will consist of the following components: I. How Healthy Are You Assessment (1 page) Students will need to use the results of their assessment to discuss why they selected the health issue as their behavior change project (see pg. 20-22 for the How Healthy Are You assessment). Remember to set ONE realistic goal that you can achieve throughout the semester (i.e. go to bed by 9PM, exercise 3 times per week, quit drinking sweetened beverages, keep a schedule to improve time management and reduce stress, etc.). II. Background of the problem (2 pages) This section introduces the health issue and provides research related to the behavior change health problem you are trying to improve and must be written in the 3rd person. Students will review the scientific literature to provide an understanding of the issues related to the problem, including how many people are affected or die from the health issue, the nature of the problem (signs, symptoms, potential complications, etc.), and potential risk factors. III. Behavior Change (2 page) Students will describe their behavior change plan. Use the SMART System to set a realistic behavior change goals that can be achieved over the course of the semester (see Steps One, Two, and Three on pgs. 14-20). For example, if your behavior change is to: lose weight, describe how much weight, your weight loss program (i.e., diet plan or exercise plan), and your short term/weekly goals. IV. Results (2 page) Students will describe how they measured their success. Where they able to follow their proposed plan Did they make changes to their weekly goals If so, what did they alter What were the successes and/or challenges Did they reach their ultimate behavior change goal If the goal was not met, what were the barriers encountered along the way, what strategies were used to overcome the challenge(s).
